CDPH: Post Survey Satisfaction Evaluation Now Available Through SurveyMonkey

On November 16, 2015, the California Department of Public Health (CDPH) announced that the current hard copy of the Post Survey Satisfaction Evaluation Form (LC-Q1) will be replaced with an electronic evaluation via SurveyMonkey.

The sixteen question SurveyMonkey is an evaluation designed to help facilities share information in order to improve the survey process. For example, one of the questions on the electronic SurveyMonkey asks “Did the survey staff introduce themselves and explained the survey process when survey began?” In this instance, the facility would then answer by rating that process from “excellent” to “poor”. However, if the facility selects a “fair” or “poor” rating, the facility will need to provide specific explanatory comments.

The majority of the questions are catered to ranking the overall survey process. The goal of the survey is to assist CDPH in improving the survey process and to provide a forum for facilities' comments to be addressed.  CDPH encourage facilities to complete the evaluation that was effective December 1, 2015. All Post Survey Satisfaction Evaluations must be submitted electronically through SurveyMonkey here. For more information regarding the Post Survey Satisfaction Evaluation Form, please contact your local district office.
